>>> HAITI DOSSIER: Focus on cholera What is Cholera? Cholera is a diarrheal disease caused by infection of the intestine with the bacterium Vibrio cholerae. Both children and adults can be infected. About 20% of those who are infected develop acute, watery diarrhea 10 -20% of these individuals develop severe watery diarrhea with vomiting. If these patients are not promptly and adequately treated, the loss of such large amounts of fluid and salts can lead to severe dehydration and death within hours. The casefatality rate in untreated cases may reach 30- 50%. Treatment is straightforward (basically rehydration) and, if applied appropriately, should keep case-fatality rate below 1%. Transmission and factors of risk Cholera is usually transmitted through faecally contaminated water or food and remains an ever-present risk in many countries. New outbreaks can occur sporadically in any part of the world where water supply, sanitation, food safety, and hygiene are inadequate. The greatest risk occurs in over-populated communities and refugee settings characterized by poor sanitation, unsafe drinking-water, and increased person to- person transmission. Because the incubation period is very short (2 hours to 5 days), the number of cases can rise extremely quickly. Prevention Prevention activities include spreading simple messages on hygiene: regularly washing hands, chlorinating or boiling water, not eating unpeeled fruits and vegetables, etc. Although new oral vaccines exist, their use in both endemic and epidemic situations requires further assessment (issues include constraints in the areas of logistics, cost, timing, vaccine production capacity, and the establishing pertinent criteria for mass vaccination). Treatment Efficient treatment resides in prompt rehydration through the administration of oral rehydration salts (ORS) or intravenous fluids, depending of the severity of cases. Up to 80% of patients can be treated adequately through the administration of ORS (WHO/UNICEF ORS standard sachet). Very severely dehydrated patients are treated through the administration of intravenous fluids, preferably Ringer lactate. In Haiti, the Ministry of Health (MSPP), supported by the WHO and MSF, has recommended setting up three types of structures to treat cholera. Cholera Treatment Centers (CTC) are typically large structures (50-100 beds), and Cholera Treatment Units (UTC) are smaller structures (typically 10-20 beds). Both do intravenous rehydration and have medical staff. It is highly recommended to isolate cholera treatment centers from other structures to avoid transmission. Oral Rehydration Points or Centers (CRO), treating moderate cases through oral rehydration salt solutions, are typically more plentiful and therefore closer to population centers (neighborhoods, small camps etc) and sometimes are managed by community members. MdM Network Activities All MdM organizations present in Haiti are spreading messages of prevention – through the radio, megaphones, posters, and information sessions at health centers and elsewhere, and most MdMs are involved with training personnel of the MSPP and other health structures. As of mid-January, MdM, often along with partners such as the MSPP, the Cuban cooperation, national and international NGOs, has set up 16 UTC, 5 CTCs, 17 CRO, and 5 ambulances that ensure the transfer of critical patients to Treatment Centers. So far 9,430 cases have been seen, with 175 deaths (1.76% mortality rate). The cholera budget of MdM Canada, Switzerland, Spain, Belgium and France combined is approximately 4.78 million USD. >>> Communication 1 year after the earthquake: all together for a success
MdM Network succeeded a new step: a real common communication. First we succeeded in obtaining common data on the action from all MdM! (thanks so much to all teams in Haiti). All MdM had the same press kit (in Dutch, English, French, German, Greek, and Spanish). We shared also the press release. 2 video films were made: 4mn in French, English and Spanish and 12mn in French. We could also

give video footage (rushes) to TVs. Beautiful photos were made by Lahcène Habib. Everything was made mainly between 20/12 and 3rd January…. A common press conference was held in Port au Prince on January 10th with participation of actors from all MdM present in Haiti and Germany. Canada, France and Switzerland held Forum discussion on Haiti. Written media, TV and radio interviews in Haiti and most countries of the network described MdM work as well as our questions on international funders and bodies. The action of the Haitian was at the centre of our testimonies.

>>> Security risk in the Sahel The security situation in the Sahel region continues to get worse. The latest kidnapping –– of two young French men in Niamey, the capital of Niger, and the rapid response by the French military which may have contributed directly or indirectly to their deaths, has pushed all MdMs in the region to reflect upon our projects: can and should we continue all of our projects? If so, where exactly? And with what security measures in place? Should we reduce the French and/or non-African staff? Would this actually reduce the risk or simply shift it to African staff? What kind of partnership should we have between MdMs in the area? In order to answer these questions, MdM France (present in Mali, Niger, Burkina Faso), along with MdM Spain (Mali and Mauritania) and MdM Belgium (Mali), are inviting certain national and expatriate staff to Paris for a week of reflection, decision-making and planning. For now restrictions have been placed on movements to the region, and MdM is maintaining a low profile. >>> Domestic programmes meeting / European Observatory on access to healthcare: On November 12th and 13th was held in Paris a big meeting on invitation of MdM France and Spain’s Presidents. 33 actors (among whom 15 volunteers/ 8 Presidents) attended the meeting and shared on past experiences and future common actions. For more info: ask for the minutes and/or look at all documents on Intranet/ Direction du Réseau International / shared files >>> The Huma network: The Public hearing at the European parliament on access to health care for undocumented pregnant women and children took place on 8th of December. It was co-organised by HUMA / MdM and PICUM, EAPN, European Womens’ Lobby. The participation of members of the parliament, their assistants and other institutions as well as NGO was very satisfying, enabling us to raise awareness among this audience on the situation of undocumented migrants in the EU. The lobbying of HUMA towards European parliament is still going on: we are trying to add amendments on access to healthcare for undocumented migrants in a report on health inequalities that is to be voted by the Parliament in March. European Declaration of health professionals: On January 17th, 5.000 health organisations, health professionals and citizens have signed the European declaration. Among others, 20 national level medical organisations, 4 European level health professionals’ organisations and 57 NGOs signed it. A decision should be taken soon on the handling of the European Declaration of health professionals: it will most probably be done to different stakeholders, in the first week of March, in Brussels. Publications: a new short HUMA publication “Are undocumented migrants and asylum seekers entitled to access health care in the EU? A comparative overview in 16 countries” is already available in English, French, Portuguese, Slovenian & Spanish on the HUMA website (in the sections “publication” and “national policies”). The versions in Czech, Dutch, German, Greek, Romanian, Swedish and Polish will be soon available (under revision). The report gathers together the executive summary of the HUMA report (2009) and results on six more countries (Cyprus, Czech Republic, Greece, Poland, Romania, Slovenia).

On 24 and 27 November, DoW UK organised 2 events on "Ensuring access to healthcare for the community’s most vulnerable migrants in the age of austerity", addressing health professionals. They already disseminated some propositions for action elaborated during the conference. On 7th of December, MdM Portugal organised its national event focusing on "Determinants of the access to health care", founded by HUMA. They launched the European declaration at the event.

>>> MdM Allemagne / Germany / Alemania / Deutschland : Exil, Exit: 12 144 visitors to ÄDW’s exhibition in Munich ÄDW presented the exhibition Exil, Exit - Living without documents in Europe on the cultural festival Tollwood in Munich from 24th November until 22th December 2010. Beyond the exhibition itself, which attracted more than 12.000 visitors, a dedicated programme with lectures, discussions and testimonies attracted more then 300 participants. It started with a panel discussion about Migration politics on EU level, to which participated Sabine Eckart (NGO medico international), Hermann Josef Hack (artist & activist), Nadja Hirsch (MEP, Liberals), Ska Keller (MEP, Green Party), Holger Kiesel (Moderation, Bavarian Broadcasting) and Prof. Dr. Heinz-Jochen Zenker (President of MdM). Olivier Jobard, photograph of the exhibition, presented his work as an introduction. Approached issues were, i.e. question of the democratic control of the activities of the EUagency Frontex through EU-institutions or the de facto obstacles to health care for undocumented migrants in Germany. The living conditions of asylum seekers in Bavaria (Bavarian Refugee Comity) The situation in the Spanish enclaves in Morocco Melilla and Ceuta (Wiliam Dräxler, Caritas) Being a MDM-Volunteer in Ethiopia, Eritrea and Gaza-Strip (Dr. Piepel and Dr. Brommundt) Lecture with Author Cem Gulay See: http://www.aerztederwelt.org/veranstaltungen/sonderseite-exil-exit.html Humanitarian Congress On 15th and 16th October, seventy experts discussed the latest developments and challenges of humanitarian aid in 20 different lectures, panel discussions and workshops. In the following some impresssions from the panels, in which Médecins to Monde participated. Obstetric Fistula: A Forgotten Illness of 2 Million Women. Experiences and Practices in the Field Under the moderation of Lecia Feszczak (MdM Germany), the surgeons Eric Peterman (MdM), Volker Herzog (MSF) and Jean-Marie Colas (MdM) presented their work. In MdM's view, this includes: Fistula surgical workshops to ensure treatment, obstetrical networks to ensure timely intervention and by that, prevention of fistula and psycho-social care to address the suffering of women living with fistula. The Role of the Military During and in the Aftermath of Natural Disasters In the panel, Dr. Jérôme Larché, member of MdM FR’s Board, advocated for a “safe distance” between humanitarian and military/political actors. Whilst dialogue remains open with representatives of the state, funders and the United Nations, MdM refuses to become subordinate to their respective agendas and as a result stays independent in its strategic and operational decision-making. Sexual Violence: Medical and Ethical Dilemmas in the Field Anne Desmarest, working as a legal advisor for MdM FR, presented our programmes and introduced a protection framework related to gender based violence which was recently published by MdM. She advocated for a comprehensive, multidisciplinary and inclusive approach, involving all relevant actors such as local and civil society organizations. Haiti Emergency, Successes and Failures - A (Self-) Critical Analysis Sophie Masson reported that successes for MdM included the ability to use and support existing structures like emergency surgical and medical teams working at the General Hospital and at primary health care clinics, the mutualisation of ressources with other NGOs and the ability to go deep in the area thanks to local staff. Emergency Surgical Care in Disaster Situations Gérard Pascal MdM FR and Paul McMaster from MSF, who both acted as surgeons in Haiti at the beginning of 2010, participated as speakers in the panel. Gérard Pascal emphasized the need for an appropriate assessment, including post-emergency planning, before starting surgical activities in emergency settings. MedMobil reports to the City of Stuttgart The team of MedMobil, with the help of the general coordinator in Munich, presents its report after 18 Months of activity in the streets of Stuttgart. The report will be presented to the responsible persons within the Citycouncil, which finances the project since July 2009. One year after its implementation, the MedMobil-bus was almost 200 times on mission, could treat 370 persons and performed more than 2800 medical and preventive actions.

Forecast: conference on access to health care for undocumented migrants in Germany in autumn 2011 Within the framework of the so-called federal work group “Health/illegality”, in which meetings ÄDW regularly takes part, a conference about access to health care for undocumented migrants in Germany is planned for autumn 2011 in Berlin. ÄDW should present during this conference its experience and practice on local and EU-level. The federal work group “Health/illegality” was founded in 2006 on invitation of the German Institute for Human Rights and the Catholic Forum “Living in the Illegality”. Its members regularly meet to discuss approaches from other European countries with the aim to develop and push up in the political debate a concept for a better health care for undocumented migrants in Germany.

>>> MdM Argentine / Argentina Comunidad toba-qom de La Primavera-Formosa "Desde Médicos del Mundo estamos trabajando en la comunidad toba-qom de La Primavera donde hemos dado atención sanitaria a 34 heridos de la represión del 23 de noviembre y a su vez estamos abordando en el territorio los complejos problemas de salud-enfermedad que padece esta población debido a las precarias condiciones de vida donde existen prevalencia de Mal de Chagas, Tuberculosis, IRAS, malnutrición, incluso heridos y traumatismos aún no resueltos entre otros perfiles epidemiológicos sumamente destructivos incluso vinculados al cultivo de la soja y uso de agrotóxicos en territorio indígena. Consideramos que avalados por la Ley nacional 26.160 y el Convenio 169 de la OIT como también por el artículo 75 inciso 17 y 22 de nuestra Constitución Nacional, la provincia de Formosa debe atenerse a respetar la propiedad comunitaria indígena de la tierras de la comunidad toba-qom de La Primavera reconocidas desde 1940 por decreto del Estado nacional, evitando así que siga primando una respuesta represiva y punitiva hacia la comunidad, dándole paso a dar respuestas integrales a las graves deudas con este pueblo desde el punto de vista del cuidado de la salud, la protección social como la reparación histórica socio-cultural aún pendientes", puntualizó en un comunicado Médicos del Mundo Argentina.

République Démocratique du Congo : lancement d’un nouveau programme triennal 2011-2013 financé par la DGD (Direction Générale de la Coopération au Développement belge) visant à appuyer 3 zones de santé du Kasaï occidental. Médecins du Monde y renforce la mise en place et l’accessibilité des soins de santé primaire.
Mali : SSP : Un programme triennal 2011-2013 financé par la DGD permettra d’appuyer 2 cercles de santé situés dans les régions de Kidal et de Mopti. L’appui de Médecins du Monde porte sur le renforcement des soins de santé primaire.
Projet enfants vulnérables : En 2011, un travail de plaidoyer est mis en place afin que l’Etat malien se donne les moyens d’une politique favorable aux droits des enfants. La nouvelle stratégie du projet voit apparaître un partenariat et une collaboration plus étroite avec les autorités maliennes du Ministère (Direction nationale de la promotion de la femme, des enfants et de la famille).
Haïti : Choléra : Le directeur sanitaire du département des Nippes a sollicité Médecins du Monde Belgique dès le vendredi 29 octobre pour une intervention choléra. Ce projet a reçu le soutien d’ECHO (700.000 € pour les six mois à venir). L’intervention de MdM comprend les volets suivants : Prise en charge médicale Mise en place d’un CTC de 40 lits pour la prise en charge des cas sévères. Mise en place de 4 UTC (unité de traitement de choléra) de 10 lits chacun pour renforcer les hôpitaux et centres de santé existants. Mise en place de 9 CRO (centre de réhydratation orale) au niveau communautaire. Formation du personnel sanitaire Formation médicale sur le choléra de 26 formateurs (1 par centre de santé) eux-mêmes chargés de former en cascade tous les agents de santé du département des Nippes (au total 550 personnels de santé ont été formées). Sensibilisation Passer par les agents de santé communautaire pour développer la prévention et la promotion de la santé en lien avec l’épidémie de choléra au niveau communautaire

Plan Hiver 2010-2011 : lancé depuis le 29 novembre en collaboration avec le Samu Social, cette action vise à intensifier le nombre de consultations médicales et paramédicales offertes aux sans-abri à Bruxelles durant les mois les plus froids (jusqu’en mars 2011). Sur le site, qui accueille pour la nuit 320 personnes, 2 à 5 bénévoles sont présents chaque soir et assurent entre 10 et 35 consultations. Au total, ce sont 60 bénévoles qui se relaient tout l’hiver, avec une majorité d’infirmiers(ères) ainsi que 5 médecins et du personnel non médical pour l’accueil.
CASO Bruxelles : En 2010, l’équipe du CASO de Bruxelles a donné 5.312 consultations (2.735 sociales, 2.222 médicales et 355 psychologiques). L’année 2010 a été marquée par la crise de l’accueil des demandeurs d’asile et par la grande inhumanité avec laquelle ces personnes ont été laissées pour compte. Dans ce contexte, l’équipe du CASO a notamment accueilli des patients en grande souffrance psychologique, accentuée par leur non-prise en charge par les services adéquats. En 2011, l’équipe va s’orienter vers un travail de formation et de sensibilisation des services de première ligne sur l’accès aux soins et sur les avantages de l’approche globale du patient pour obtenir un accès aux soins effectif.
Evénements/campagnes : Soirée dans la librairie Filigranes (9 décembre 2010), en présence d’Eric-Emmanuel Schmitt : 25% de la recette de la soirée a été versée à MdM, soit 7.200 €. Lancement du « Brussels-to-Paris Biketour 2011 ». Le principe : 350 km à vélo en 3 jours pour soutenir les projets humanitaires de Médecins du Monde. Un départ de Bruxelles le 14 juillet 2011 avec en point d’orgue, l’arrivée à Paris le 16 juillet en même temps que les cyclistes du « London-to-Paris ». Chaque participant se fera parrainer par son entourage : amis, familles, collègues. Plus d’info sur le blog. 20 km de Bruxelles, 29 mai 2011 : En 2010, plus de 200 personnes ont participé aux 20 km de Bruxelles pour récolter des fonds pour MdM. Résultat : 26.000 €. Le 8 décembre 2010, Médecins du Monde Canada a célébré le dixième anniversaire du projet Montréal qui vise à rendre accessible les soins de santé aux plus démunis dans la grande ville de Montréal. Lors de cette soirée, les intervenants et partenaires du projet sont venus témoigner du travail exceptionnel fait par toute l'équipe du projet Montréal et surtout de ses retombées dans la communauté.
Afin d'exprimer son soutien à la communauté haïtienne du Québec lors de la commémoration du séisme du 12 janvier, Médecins du Monde Canada s'est rendu à la Maison d'Haïti afin d'observer la minute de silence à 16h53 et partager ce moment de grande émotion avec la diaspora haïtienne.
En plus de continuer ses projets en Haïti, MdM Canada est présent au Nicaragua, en République Dominicaine et en Colombie.
>>> MdM Espagne / Spain / España : XIV Premio Luis Valtueña El fotógrafo vasco Fernando Moleres se ha alzado con el XIV Premio Internacional de Fotografía Humanitaria Luis Valtueña por su serie fotográfica ‘Niños prisioneros en cárceles africanas’ sobre la situación de los menores encarcelados en las prisiones de Sierra Leona. La lista de premiados la completan el también español Javier Arcenillas y el fotógrafo italiano Riccardo Venturi. Las imágenes premiadas y seleccionadas conforman las 40 instantáneas que darán forma a la exposición de esta edición del premio y cuyo recorrido itinerante dará comienzo en la Alhóndiga Bilbao el próximo 20 de enero. El primer premio obtenido por Fernando Moleres consiste en una beca de trabajo de 8.000 euros destinada a la realización de un proyecto fotográfico en una de los ámbitos de trabajo de Médicos del Mundo. Médicos del Mundo alerta de la crisis de personal sanitario El Informe 2010 “La salud en la cooperación al desarrollo y la acción humanitaria”, realizado por las ONGD Médicos del Mundo, Prosalus y medicusmundi, refleja que en 2009 comenzó la disminución de la Ayuda Oficial al Desarrollo (AOD) por parte del Gobierno central y las Comunidades Autónomas.

Se trató de la primera reducción en siete años y supuso sólo el preludio de la considerable reducción que, sin duda, se producirá en 2010 y en 2011. Así, de acuerdo a los Presupuestos Generales del Estado para 2011, el próximo año se producirá el mayor retroceso de la AOD en España, con una reducción casi un 20 % inferior a la de 2009 y 2010 según ha señalado la Coordinadora de ONGD de España. El porcentaje de la renta nacional bruta destinado a AOD quedará en el 0,40%, situándose así lejos del 0,7% asumido para 2012 por el Gobierno y por el resto de partidos políticos con representación parlamentaria que lo habían apoyado a través del Pacto de Estado contra la Pobreza. Plano estratégico 2011/2014 Los próximos 29 y 30 de enero, se celebrarán las Jornadas de Voluntariado de la asociación, que, en esta ocasión, servirán de escenario para reflexionar y trabajar en el nuevo Plan Estratégico. Las jornadas serán participativas, como espacio de encuentro, reflexión y debate, en el que se trabajará por líneas estratégicas. Encuentro Centroamérica Del 1 al 3 de diciembre tuvo lugar en León, Nicaragua, el primer encuentro centroamericano en el marco del convenio financiado por AECID-MdM: mejora de las condiciones de ejercicio del derecho a la salud de las poblaciones en situación precaria, con especial énfasis en adolescentes y mujeres, personas en situación de prostitución y poblaciones móviles en El Salvador, Guatemala, Nicaragua y Honduras. Esta reunión persiguió, entre otras cosas, relanzar los espacios de coordinación y debate que se mantenían en la zona durante el desarrollo de un programa de salud sexual y reproductiva en los cuatro países, que finalizó en 2007. VIII mesa de coordinación en el Sahara Del 5 al 7 de diciembre se celebró en los campamentos de personas refugiadas saharauis de Tinduf, la octava edición de la Mesa de Concertación y Coordinación en Salud, uno de los principales componentes del convenio del Sahara, donde dos veces al año, una en España y otra en Argelia, los distintos actores que trabajamos en los campamentos en el sector salud alineamos y concertamos esfuerzos. Inauguración de una clínica materno-infantil en Honduras MdM ha inaugurado en diciembre una clínica materno-infantil en el municipio hondureño de Orocuina, con un presupuesto de 98.013 euros y la colaboración de la AECID, autoridades de Salud hondureñas y la Alcaldía. La nueva clínica, y el centro de salud recién remodelado, atenderán las necesidades de la población de los municipios de Apacilagua, Orocuina y Morolica (departamento de Choluteca) y Soledad y Liure (El Paraíso), donde uno de los mayores males existentes en el área de salud es la mortalidad materno-infantil. >>> MdM France / Francia :
Nouveau projet de Réduction des Risques auprès des usagers de drogues à Tbilissi en Géorgie
Nouveau projet, au Laos, dans deux districts de la province de Champassak, pour la santé sexuelle et reproductive.
Une mission contact ciblant les besoins de la Minorité Yi en termes de Réductions des Risques et de prévention HIV auprès des usagers de drogues, en Chine, dans la province du Shishuan (Comté du Liangshan) à été menée en décembre. En fonction des résultats , une mission exploratoire pourra être envisagée.
Liberia : Près de six ans après la fin de la guerre civile, le Liberia poursuit sa reconstruction. Malgré une forte volonté politique, le système sanitaire reste fragile et la contribution du gouvernement au secteur de santé s’avère limitée face à l’ampleur des besoins. Dans la continuité de ses actions au Libéria, MdM vient d’achever la construction de l’hôpital CB Dunbar dans le comté du Bong, fruit d’un partenariat fort avec le Ministère de la Santé et du soutien financier de la fondation McCall Mac Bain. Construit en 1952, l’hôpital est détruit pendant la guerre (1989-2003). Suite à ce conflit qui a couté la vie à 200 000 personnes, MdM démarre ses activités dans le comté du Bong en 2003 avec un programme de rétablissement de l’accès aux soins des populations rurales. La reconstruction de l’hôpital permet aujourd’hui de compléter l’offre de soins par une structure spécialisée en gynéco obstétrique et néo natalité. Avec un taux de mortalité maternelle de 994 sur 100 000, la santé reproductive représente un enjeu majeur du système de santé libérien. L’hôpital intégrera également le centre de santé et le service d’urgence existants. Cette structure deviendra un centre de formation national en gynéco obstétrique. Inauguré par la présidente du Libéria, Ellen Johnson-Sirleaf, en décembre, l’hôpital démarrera ses activités dans le courant du mois de février.
Transfert du Programme d’Echange de Seringues (PES) d’Angoulême à l’association AIDES au 31 décembre 2010. L’équipe MdM travaillait avec AIDES depuis juin 2006 : le partenariat prévoyait initialement la mise à disposition par MdM d'un animateur de prévention à 0.5 etp, augmenté à 0.8 etp en

2007, la participation des bénévoles MdM et la mise à disposition de matériel de prévention. Les interventions en unité mobile étaient effectuées avec le bus MdM jusqu’en mars 2010. Depuis, le soutien de MdM au PES consistait principalement en la mise à disposition de l'animateur de prévention. Toutes les autres composantes organisationnelles étant gérées par Aides. MdM a souhaité se désengager de ce partenariat laissant ainsi Aides piloter entièrement ce projet. C’était le dernier programme d’échange de seringues de MdM France. Programme Education aux risques liés à l’injection : Ce programme très innovant et à la limite de la légalité s’est doté d’une unité mobile neuve fin 2010 et a démarré des sessions hors les murs en janvier 2011. L’équipe est constituée d’une coordinatrice, d’un infirmier et d’un éducateur à la santé. A ce jour, 2 sessions ont eu lieu et ont permis d’inclure 3 usagers dans le programme. Combat inter associatif pour la protection des étrangers malades Les 20 associations réunies dans le collectif Observatoire du droit à la santé des étrangers (ODSE), dont MdM, tentent de sauvegarder les 2 dispositifs d’accès aux soins et de protection : l’Aide Médicale Etat (AME) et le droit au séjour des étrangers gravement malades qui ne peuvent être soignés dans leur pays d’origine. Une mobilisation sans faille a permis de faire refuser par le Sénat les textes proposés par le gouvernement et certains députés. En revanche, l’Assemblée Nationale a d’ores et déjà voté une contribution de 30 euros annuels par adulte à l’AME ainsi qu’une restriction des soins les plus lourds en imposant un contrôle administratif à l’hôpital. La possible réforme (négative) du droit au séjour pour soins sera examinée et votée en février. >>> MdM Grèce / Greece / Grecia / Ελλάδα:
Action and survey: In the frame of the year dedicated to poverty and social inclusion, we organized two actions which included two seminars, the first referred to the socially excluded groups of people and the information about their rights in the sector of health, education and work and the second one referred to the intercultural education of the employees who work with vulnerable social groups. The third action was a survey entitled: “The access to health, education and work of poor and socially excluded groups of people who live in the center of Athens”. In addition, we made information brochures for the rights of these groups of people and we published them in five different languages as well as an information leaflet with all the services and organizations to which the immigrants can address.
Roma : On 12th of January, MdM Greece organized a day conference about our program “Roma – Mobile Unit” entitled “Supply of Primary Health care treatment to Roma populations in Greece”.
Uganda: On the 15th of January a team of 5 volunteer doctors departed to Uganda. There, along with their medical expertise the team will provide medicines to the patients of the clinic of Monde and to the residents of the surrounding areas.
Tanzania: Since September 2010, MdM-Greece is operating a Diabetes Clinic in the village of Ifakara. The number of patients is growing on a monthly base and it is planned for 2011 to begin the functioning of a Diabetes Mobile Unit, in order to cover the needs of the remote areas.

Tokyo Project End of Year Activities: At the end of 2010, the number of people living in the streets followed on a regular basis by Tokyo Project reached 50 people. Special activities were organized between December 29 and January 3 to compensate the fact that all public services traditionally shut off during the end of year period in Japan. Each day, between 4pm and 6pm, socio-medical consultations were given to homeless people in East-Ikebukuro Central Park, 20 people being taken care of by the team of volunteers led by the salaried coordinator Azusa Nakamura. One session of Social Skill Training targeting homeless people and one session of education for volunteers were also organized during this period.
TV Program on Tokyo Project : A 30-minutes program on Tokyo Project was shown on NHK Educational TV in September.
Opération Sourire in Bangladesh : From November 25 to December 2nd, a team of 7 Japanese volunteers (3 surgeons, 3 nurses, 1 coordinator) went to Bangladesh to perform surgical operations as part

of Operation Sourire. From France, 2 anaesthetists joined the team. For the first time, volunteers from MdM UK and MdM Netherlands took part to the mission organized by MdM Japan. The objective for these associations was to get a first practical experience of Operation Sourire, in order to be able to organize this type of mission from London and Amsterdam in the future. Moreover, this mission was the first Japanese one to be funded through the partnership concluded between MdM France and L’Oreal for the Operation Sourire.
Operation Sourire in Cambodia : A Japanese team comprised of Dr Terashima, Dr Morioka and Ms. Ishihara went to Cambodia from October 17 to 23 and performed 36 surgical operations. Two anaesthetists from France, Dr Nguyen and Dr Chassery, joined the team, coordinated by Nao Kuroyanagi, MdM Japan Missions Manager.
TICAD Advocacy Meetings : The Japanese government organizes every 5 years the Tokyo International Conference on African Development (TICAD) gathering the heads of state of African countries and international organizations such as the World Bank and UNDP. In view of the next edition in 2013, some Japanese NGOs belonging to the GCAP (Global Call Against Poverty) coalition have started a series of meetings with the different stakeholders to try to influence the outcome of the summit. The objective is to make sure the TICAD process serves the interests of the most vulnerable populations in Africa, and that the civil society, both in Africa and in Japan, are associated to the process. Since July, MdM Japan, as a member organization of this TICAD Advocacy Group inside GCAP Japan, participated to meetings about the TICAD process with the MOFA, JICA, the Ambassadors of DRC, Egypt, Mali, Djibouti, Tunisia, Tanzania.
« Congo Week » in Yokohama: During one week in September, the shopping centre « Tressa » in Yokohama welcomed a small photo exhibition on the Democratic Republic of Congo (DRC) linked to the congophone campaign (www.congophone.com) that also gave MDM Japan the opportunity to hold a special street fundraising campaign inside the shopping centre premises.
Symposium on « How the Japanese media talk about the MDGs »: The Japanese NGO network GCAP Japan (Global Call Against Poverty Japan) organized a symposium on September 8, to discuss the relations that NGOs have with the media, and how the Japanese media reflect on MDGs and development issues. Yu Kumano participated as a speaker to present the NGO view on the question, together with a panel of journalists.

MEDOC ‘Plus’ 2010 General goal is to fill niches of the regular Dutch healthcare for undocumented, by providing information and mediation, drugstore medicines and diagnostic tests, prevention activities, regular registrations with general practitioners and a direct available network of other caregivers (dentists, physiotherapists) The program opened in November 2011 with a new coordinator. Location is the centre ‘Step Further’ in Amsterdam ZuidOost, owned and coordinated by a partnership of 6 churches. MdM NL rents 3 days a week office space to carry out activities. The centre is opened 1 morning per week for patients. In Nov/Dec there have been 16 patient contacts, 5 MEDOC-booklets and 3 reported incidents. Most questions concern the difficulty of accessing a hospital (they do have referral note from a general practitioner) for financial reasons. Most of the patients repeat their visit to our centre with other questions or for continuation of the started mediation (on access to healthcare). Most are from Ghana and Nigeria, and often already for a longer time in NL. A stock of drugstore medicines (available without doctors prescription) and tests (to detect pregnancy, diabetes, blood pressure etc.) has been obtained, in order to help patients (if needed) pending their appointment with the general practitioner.
>>> MdM Portugal Action of sensibilisation to fight acute diarrhoea in Island of Mozambique Following an outbreak of acute diarrhea in Mozambique Island - a subject that has concentrated much of the attention of local health professionals, from last July to September - MdM Portugal has supported two sessions of health education. These actions consisted of production of posters for hospital rooms and dissemination of hygiene practices. Complementing these, a technician was invited to participate in preventive medicine radio program, in English, "Child Life Expectancy in the Community" on September 11 and a program in Makua - the local language, on September 12. The programs were issued on weekend that coincided with the day of Eïd (which marks the end

of Ramadan). Because of this, the program had larger audience. There was talk about what are the Acute Diarrhoeal Diseases (ADD), and what preventive care. Furthermore, there were phone calls with questions and additional information, particularly by nurses from other districts. In the end every child who attended the program at the studio had to repeat some of the prevention methods and gave own contribution to the program (through a song…etc)

Disaster Risk Reduction in Bangladesh: On 1st February Doctors of the World’s exploratory mission for our first overseas project gets underway.
Last year Doctors of the World UK & Project:London submitted evidence for the UK government’s review of the National Health Service. The government’s Health Bill is to be released later this week but it is currently causing much debate in the medical profession. It remains to be seen what the major reforms will mean for us.

UDM right to health care: On November 26 MdM Sweden was one of the organizations that hosted a conference in Stockholm regarding UDM right to health care in Sweden. The purpose was to raise awareness in the issue and create a public debate and opinion. Furthermore its purpose was to provide a natural meeting place for politicians, health care professionals, researchers and other involved persons in order to create conditions for building a common platform. There were many speakers for example Anders Björkman (former president of MdM SE), Nathalie Simonnot from the DRI and Paul Hunt UN Special Rapporteur on the right to health (2002-2008).
Give health care to all people: In December MdM Sweden arranged a symposium at the Medical national meeting in Gothenburg (Swedish medical association (SMA) was the host). Our president My Morin sat in a debate with MP’s from the conservative and the liberal party, the leader of the ongoing enquiry and a representative from IFMSA. The president of the SMA was the moderator. My Morin encouraged the audience and all her colleagues to civil courage ie give health care to all people including UDM. This gave her big applause.

Haïti : Suite à l’épidémie de choléra apparue en octobre 2010, MdM a rapidement réagi afin d’éviter une flambée de la maladie en mettant sur pied un Centre de Traitement du Choléra (CTC) comprenant 85 lits dans la région goâvienne en partenariat avec les Croix Rouges Norvégienne et Suisse, Terre des hommes Lausanne et Intermon Oxfam. Ce centre a permis de prendre en charge les cas modérés et graves de choléra 24 heures sur 24 et de ralentir la croissance de l’épidémie. Le 12 janvier 2011 a marqué l’année qui s’est déroulée depuis le séisme. A cette occasion, MdM Suisse a organisé une semaine de commémoration afin de faire le point sur l’évolution de la situation en Haïti. Un colloque s’est tenu à Genève organisé par le Centre d’Enseignement et de Recherche Humanitaire (CERAH) avec pour invitée une infirmière haïtienne travaillant pour MdM venue témoigner du travail effectué sur place. Une soirée-débat et un cocktail haïtien ont été organisés à Neuchâtel le 13 janvier afin de parler de l’aide à la reconstruction et du rôle des ONG. Le Pr. Nago Humbert a tenu le rôle de modérateur, animant la discussion des intervenants représentés par Félix Bollmann, directeur de la Chaîne

du Bonheur, Pierre Salignon, directeur de l'action humanitaire de MdM France, Marie-Lourdes St Felix, responsable de la santé communautaire de MdM Suisse en Haïti nous ont fait l’amitié de leur présence.
Permanences Blanches : Le mois de février va marquer la reprise du projet des Permanences Blanches, en collaboration avec l’association Fleur de Pavé, après un an d’arrêt. Ce programme vise à offrir un espace d'écoute et de prévention concernant les questions de santé auprès des travailleurs et des travailleuses du sexe à Lausanne. Les permanences se tiendront le soir dans un bus selon le principe de l’accueil à bas seuil. Il s’agit en effet d’écouter les problèmes et soucis de ces personnes afin de répondre de manière ciblée en matière de santé. De même, une permanence diurne sera effectuée et proposera des séances d’information sur la santé et les droits à la santé.
Territoires palestiniens : Le PNUD a décidé d’étendre le financement concernant le projet de prise en charge de santé mentale des enfants et des adolescents palestiniens jusqu’en juin 2011. Celai permet à MdM de poursuivre et de consolider ses activités, ainsi que de préciser les orientations du projet pour le futur. Le plan de formation se poursuivra ainsi que le renforcement des activités de santé communautaire par le biais d'actions de prévention et de sensibilisation à la santé mentale. Le 18 novembre 2010, MdM a organisé une conférence sur la situation israélo-palestinienne au Club 44 de la Chaux de Fonds. L’invitée, la conseillère fédérale Mme Micheline Calmy-Rey, cheffe du Département des affaires étrangères s’est exprimée sur le rôle de la Suisse dans le conflit et le Pr. Nago Humbert a exposé le point de vue et le rôle des humanitaires dans ce genre de contexte d’intervention. Le public a pu s’exprimer librement en posant des questions aux deux intervenants.
Bénin : Le 4 novembre 2010, Nago Humbert, Président de MdM Suisse, s’est rendu à Abomey pour l’inauguration de l’ouverture d’une première antenne de prise en charge de la drépanocytose. Celle-ci est désormais opérationnelle et accueille régulièrement les patients atteints par la maladie. Cette ouverture représente la consécration du premier volet du projet. La suite s’articulera autour d’une campagne d’information, d’éducation et de communication dans la région du Zou et des Collines pour atteindre les familles atteintes de drépanocytose. Une modélisation du projet est en cours afin d’envisager la mise en place de ce service dans d’autres régions du Bénin.
Cameroun : Un consultant engagé par MdM Suisse mène actuellement une mission exploratoire au Cameroun dans la commune de Koupa Kagnam au sein d’un Centre créé par l’ONG REA Cameroun. Il s’agit d’y évaluer les besoins liés à la santé et analyser dans quelle mesure il serait possible d’intégrer un volet médical à ce centre éducatif qui accueille environ 400 enfants et leur famille.

Ils souffrent du chômage, de discrimination et de manque d'opportunités à l'école et dans le monde du travail… Ils accusent les immigrés de violer les valeurs européennes. Mais trop souvent, ce sont ces accusateurs qui corrompent ces valeurs et ainsi le concept même de citoyen de l'Union européenne. Les chapitres les plus sombres de l'Europe ont été écrits dans un tel langage. Aujourd'hui les cibles principales sont les immigrés de foi musulmane. L'Europe ne peut pas se permettre les clichés qui ferment les esprits et nourrissent la haine. Et le monde ne peut pas se permettre une Europe qui agit ainsi », a affirmé Ban Ki-moon. In a statement revising its policy, the WMA said that refugees, including asylum seekers, refused asylum seekers and undocumented migrants, as well as internally displaced persons in all regions were among the most vulnerable in society and international codes of human rights and medical ethics declared that all people were entitled without discrimination to appropriate medical care. Livres à lire : De la violence à la persécution, femmes sur la route de l’exil, de Smaïn Laacher, éditions La Dispute (12€) : 69 entretiens majoritairement avec des femmes permettent de décrire les violences pendant le parcours migratoire. En dehors des destins individuels brisés par la répétition des violences, c’est le côté systématique, comme accepté, dit « normal » des violences sexuelles sur les femmes qui est effrayant : un livre à lire pour se rappeler de l’importance de donner aux femmes rencontrées sur les programmes (mais les hommes ne sont pas toujours épargnés…) un espace où tenter de reconstruire, d’intégrer la non acceptation de ces violences.
La prise en charge des troubles mentaux facilitée L'Organisation Mondiale de la Santé (OMS) vient de rendre disponible sur son site Internet un nouveau guide de recommandations pour la prise en charge des troubles mentaux, neurologiques et liés à l'utilisation de substances psychoactives. Ce Guide d'intervention a pour objectif de faciliter la prise en charge de la dépression, des troubles dus à la consommation d'alcool, ou encore de l'épilepsie. Il devrait permettre au personnel médical non spécialisé de posséder les compétences nécessaires pour poser un diagnostic et prendre les patients en charge.
